When someone is considering getting a dental implant to replace a missing or decaying tooth, one of the most important factors is jaw bone density.

That’s because the implant (tiny metal post) will be placed in the gum where it is supposed to fuse with the jaw bone, which needs to grow around it. The jaw bone needs to be strong and dense to hold a dental implant and a replacement tooth (crown).

If a patient lacks a thick jaw bone due to periodontal disease, injury, or infection, then a bone graft will be needed before dental implant surgery happens. A bone graft replaces and regenerates the lost bone. A bone graft creates a more solid base for a dental implant, which can last a lifetime.

While bone grafting may sound a little scary, it’s actually a very common procedure for dental implants and other medical needs. Bone grafting may include (1) removing a very small piece of bone from another part of the patient’s body, or (2) using a small bone from a cadaver or (3) an animal source. A cadaver is usually the easiest choice because it prevents the need for a second surgery to remove a piece of bone from the patient’s body.

During bone grafting, the oral surgeon opens up the gum, and places the small extra bone on top of the jaw bone. The piece of bone should fuse with the jaw bone over a few months. The oral surgeon then checks the jaw bone with high-tech equipment to see if the bone density has improved enough for a dental implant. If the density is not thick enough, a second bone graft may be needed.

Bone grafts can be time-consuming, but are a necessary part of the dental implant process. After a bone graft procedure, patients may have minor swelling or bruising, but usually over-the-counter pain medications are enough to deal with any pain. Patients should only eat soft foods while their mouth heals.

When the healing is complete and the jaw bone is dense enough, the dental implant will be surgically placed into the jaw bone where it will fuse with the bone. The bone will need several months to heal. Once that happens, an abutment is placed on the implant, and then a crown is screwed on top of the abutment.
