There are times when a tooth is damaged beyond repair by infection or injury, and needs to be replaced with a dental implant.

Before the dental implant can be put in, the bad tooth needs to be extracted (pulled). Tooth extractions are a common procedure in dentistry, and are not nearly as scary as people may think.

The gum is given plenty of painkiller before the extraction, and there is also time for the painkiller to take effect. The extraction itself should not cause you pain, although you may feel some pressure during the procedure.

If you plan on getting a dental implant, it’s a good idea to get a bone graft immediately following the tooth extraction. That’s because the tooth leaves an open socket inside the bone that once supported it, and new bone doesn’t normally grow inside the empty socket. In fact, the bone will usually shrink around the extraction area. Without enough bone, there is nothing to support your future implant.

A bone graft is made from bone taken from another part of your body, cadavers, or an animal source. Cadavers are the most common choice. A ridge preservation bone graft can be placed in the empty gum socket right after the tooth extraction to provide a foundation for the dental implant. The bone graft procedure should not cause any pain after the extraction.

Following the extraction and bone graft, the gum will need to heal for about three months before a dental implant can be placed. This wait time is important because the jaw bone needs to be dense to support the implant. It is important to keep your tooth extraction/bone graft area clean. You do not want an infection, which can lead to pain, swelling, and additional surgery.

After you have had your tooth extraction and bone graft, stitches will be placed in your gums to minimize bleeding and help healing. Over-the-counter medications should be able to handle any pain you might experience afterwards. Some stitches are dissolvable and will simply go away. If your stitches are not the dissolvable kind, we will remove them about one week after surgery. Having your stitches removed may feel odd, but should not cause you any pain.

Once the bone graft has succeeded, our oral surgeon will place the dental implant (tiny metal post) into the jaw bone where it will fuse with the bone for three months. Following that wait time, an abutment will be placed on top of the implant, and the crown (artificial tooth) is screwed on to the abutment. Only the crown will be visible, as the implant is below the gum line.
